the seven fractures story

just now i extracted my lower left 8 after 2 weeks extracting the right side. It was Dr Yanal who done that job.

whew.

no pain, but its damn scary. little pain for LA. The scalpel hit my mucosa, tear it and i can hear the bone been hit by instruments. My legs streched, my heart beats nearly to explode and my eyes shut. But i make no sound or excuses...i want these to end asap.

I guessed that Dr used elevator to extract my tooth, and everytime he used that, i heard 'crack' ang A LOT of pressure i felt like i dont have ramus anymore.

Somehow i managed to open my eyes to tell Dr that im not fainted or whatnot, haha, and i saw drops of blood at wajah suci Dr. sorry. And that tells me how hard it is to take out my tooth and how much blood in there. See the pics below, sengaja pakai white coat to see how much blood it is. Drops of blood through my whitecoat.

The procedure done after about 13minutes, alhamdulillah... and i took the chance to see my tooth. i counted, they're about 7 large fractures. I heard them talking while extracting my tooth :

' fi kaman remaining root fractures inside?'
Dr yanal : 'yes, maybe thousands of them'

uhhu...

and i have to use ice pack and attend next appointment for their research about inflammation.

'salamatik ya nur'...yes, thanx Doc.

jaw registration, try in

yesterday i had prosthodontics clinic, and i'm the assistant.

My partner did jaw registration..these are few things that are important to be done during jaw registration :

1) check naso labial fold, should be about 90 degree. If not, should trim the wax.
2) labial fullnes, same as above
-check the plane wit fox plane...correspond with ala tragus line, and interpapillary line.
3) height of the record block should be 0-2mm from the lip.
4) check the smile line, its the line where teeth end? maybe..
5) vertical dimension. Vertical dimension on occlusion (VDO) minus vertical dimension on rest (VDR) should be about 0-4mm. The patient shouldn't be stress during this procedure the get the correct measurement.
6) mark incisal line(correspond with lip for esthetic?) and canine line which is a line from corner of eye, thru ala of nose.(middle of canine) If its distal...when patient smile, corner of mouth.
7) prepare the upper recor block: removing wax .
8) check if there's anterior open bite or posterior.
9) ask patient to bite on molar, relax and put tongue on roof of mouth. Register his jaw. Patient shouldnt open mouth. Melt the wax between upper and lower. open the lower denture first then remove upper n lower together from the mouth.
10) put sticky wax between them. make sure corner of the cast at TMJ site dont touch each other, coz if they touch each other, acrylic will not be correctly positioned in cast and everything will be wrong.

Try in

1) check everything, sound, esthetics, if theres pain when wearing it....retention isnt important coz there's no post dam.
2) mark post dam area ( the nonmovable part of soft palate-theleast movable coz soft tissue always move)
few techniques to take post dam area :

- nonmovable part os soft palate
- valva maneuver - blow over closed nose, soft palate will go down while har palate not, so mark the junction between them
- wax that will harden in mouth, but still can apply pressure on soft palate.
- apply burnisher at soft palate, run it until you feel a drop, mark there.

put denture in, post dam area will be marked at the acrylis, trim away the excess, then mark on cast to carve away the shape of pos dam area.

a dentist, a publisher, a promoter?!

Huh, sejak akhir-akhir ni hidup aku jam. Komputer, otak, dua-dua jam. Masa pun jam juge.

I'm a dentist who works with people's teeth. I take radiographs, do restorations@fillings, extract teeth, scaling, examinations and give advises about teeth.

Dalam pada itu, aku perlu menyiapkan a few requirements, which are partial denture and RCT.

= saya pelajar pergigian, saya buat gigi palsu separa di pusat kesihatan. Awak tahu sesiapa nak buat gigi palsu? Kalau ada bagitau saya ya. Saya kerja hari isnin'

ohho...ada sekali tu nampak pakcik takde gigi depan. Terus gi tanya dia nak buat gigi palsu tak? (adus betapa aku teruja tengok orang takde gigi, bagai rezeki melimpah). Dia dengan malu dan tidak selesa kata yang dia buat dengan doktor sendiri.

Hadus, itula aku, lupa untuk menjadi sensitif. Gigi adalah sensitif ya. Terus dah tak nampak gigi dia yang takde tu bila cakap ngan dia. Menyesal.

Begitulah aku 2-3 hari ini, memburu patient. Mana taknya, kalau tak cukupkan requirement, tak pass! Wal 3iyaz biLlah.

Next is a publisher. Not really a publisher, i did some graphics, i arranged pages which are 70 okay...jammed! Printing, photostating, cutting, arranging again..went to publish them at balad and had to discuss about everything....whoa. Go back, discuss about price, pages, sizes naaa....

My computer jam because of graphic'ing, my otak jam because of..everything la.

today, or i can say tonight, i gotta study about partial denture coz just now Dr Mahmud said you should know theoretical knowledge, and i teach you practical. Okay...

kesusahan

kesusahan, perit dan jerih yang kita alami adalah untuk dilupakan rasanya, dan diingati manfaatnya.

Bagi saya, kesibukan atau tambahan kerja yang perlu saya buat, perlu saya simpan dan tidak bercerita mengenai kesusahan itu, sebab semakin saya mengingati kesusahan itu, semakin lah terasa susah. Contohnya suatu beban atau kerja yang perlu disiapkan dalam tempoh tertentu dalam masa sama saya juga perlu tumpu pada pelajaran.

Saya tidak bercerita pada orang terdekat mengenai kesusahan itu, kerana jika mereka tahu saya sibuk, dan saya sendiri dilayan seperti orang sibuk, dan saya diberi tanggapan sebagai orang sibuk...semakinlah menjadi-jadi kesibukan dan pressure yang saya perlu tanggung.

Begitu psikologi yang saya gunakan, mungkin boleh digunakan orang lain, mungkin sesuai untuk saya seorang sahaja.

Bagi saya, penting untuk kita melatih otak kita untuk melupakan beberapa perkara demi kebaikan sendiri juga :

- kesusahan yang menyusahkan jika mengingatinya
- kebaikan yang kita lakukan
- kutukan dari manusia yang berniat salah
- kisah2 buruk tentang seseorang yang kita dengar kerana boleh jadi kita bersangka buruk kepada seseorang walau sebenarnya kita belum mengenalinya

about patient

yesterday i had pedodontics clinic. Well, my patient is reallly cooperative, she even translates what i said to my partner. GOOD.

Monday is holiday, maulud nabi.

On Sunday, i had radiology clinic. I made two extraoral radiography which is a blessing coz i never do it before, and thats the reason why i got low marks for clinical assesment last semester. Mind you, that my course is continuos from last semester so i can compensate my marks last semester with this semester.

Taking panoramic radiography is just easy! huaha..Firstly, ask the patient to remove necklace, earings, pins or any metal things she's wearing.

Then, press the button 'return' to let the machine return to its place so we can put the film. Then put the film with the logo 'tube inside' facing outside.

Adjust the height of the machine so its suitable with the height of the patient. Put his/her chin on the chinplace and tighten the one that hold the patient's forehead so that his head dont move. (before that, check that the line of light is at midline of the face and parallel with frankfort plane)

Ask the patient to bite on his/her incisors, put tongue on roof of mouth, then go out to make exposure. One of the button should be 16(???) and then press exposure until the light turns on and turns off, then u can remove your finger. (the machine should move one round, then it will return by itself by moving to the opposite direction. Take the film, go to processing.

Close all lights, and doors. Press red button for fast processing.(only for extraoral Rg) Open the case (there's only one layer that is the film). Open the processor, put the film vertically from your direction, hold it until it goes inside, close the processor. Put new film in the case. And then just wait until the film is ready. While its still processing, dont turn on lights okay.

Yes, happy!

Heh...its the same process for lateral cephalogram but the film and machine is different. And there's one button should be '97' fot Lateral Cephalogram.

There goes this 'mutakhalluf' girl or 'budak tak reti' . (mutakhalluf : budak yang dari ulu-ulu gitu, tak reti sangat!)huhu...jahat gile aku.

Mane taknye geram, we treat her very nicely. She should know that she came here, for free. She's treated for free. She shouldn't be that choosy! She dont want me to take her radiograph coz i looks different, not arab yeah right! Banyak songeh ko kate ade darah la, kitorang wat saket la..wey! Aku kemain semangat nak terangkan kat dia bebaik ini radiograph cemane, die buat hal. No no... aku blah gitu tinggalkan die, yep you deserve that. Kalau nanti aku ade klinik ke...ada patient gini, aku just cakap, 'please, out!'

Salah satu daripada impianku dalam hidup ini adalah mencapai kesejahteraan dan kebersamaan dalam agama. Merasa cinta dan tenggelam dalam asyiknya ilmu Ilahi mengenai agamaNya Islam. Itu aku cukup pasti telah ada di hadapan mataku pada diri seseorang yang bernama ayahku.

Tanpa satu pun usaha dariku, salah satu dari tujuh warna itu telah ku capai, warnanya putih. Itu adalah warna pertalian darahku dengan ayahku. Putih. Seolah warna yang enam lagi jauh sungguh dari aku.

Ungu - warna-warni hidup seorang huffaz yang ayat-ayat Quran menjadi penyeri hidupnya sepanjang waktu

merah - Warna kesihatan yang baik sepanjang merasakan batang tubuh milik Allah

Jingga - Kesungguhan menuntut ilmu dan kecintaan kepada ilmu

Segala susah payah yang saya tempuhi, hujungnya mesti ada ganjaran berupa kejayaan.
Begitu juga belajar di luar negara, sudah pasti bukan senang. Belajar mana-mana pun susah, tapi dalam situasi diri saya sendiri, balasan kerana bersusah payah belajar di luar sedangkan boleh juga belajar di dalam negara, mestilah lumayan.

Ini janji saya pada diri sendiri, saya mesti belajar bersungguh-sungguh. Alang-alang menyeluk pekasam biar sampai ke pangkal lengan. Alang-alang bersusah-susah ke luar negara, biar sampai berjaya.
